INTRODUCTION
When an employee elects to resign from their current position, their immediate supervisor must follow these guidelines to ensure a smooth and efficient off-boarding process. This includes completion of the Employee Dismissal Form.
The purpose of this form is to notify HR of an employee's resignation. Not adhering to this procedure may cause delays in the processing of the employee’s final paycheck.
NOTE: Acceptable method of resignation are: formal resignation letter or email sent to supervisor. Unofficial channels of communication, such as group text messages, Qliq, or verbal communication are not acceptable.
PROCESS
1. Resigning employee submits a Resignation letter complete with last day of service to his/her supervisor.
2. Immediate supervisor to open a ticket and to select the Employee Dismissal Form from the ticket for and complete this within (1) business day upon receiving intent from a resigning employee.
3. Upon receipt of the Employee Dismissal Form ticket, HR will send the Direct Deposit and Employee Acknowledgment form to the supervisor to fill out with the employee, which must be completed and submitted no later than (2) business days prior to resigning employee's final day.
4. The final paycheck will be processed no later than one business day after the employee's final day. IT will also deactivate all accesses of the resigning employee as soon as possible after the last day.
